Narrative:

Dispatched from ZZZ with single left pack; right pack inoperative documented with placard in logbook. Taxi out and takeoff normal. At 2000 ft entered cloud layer with icing conditions; turned on engine and airfoil anti ice systems. Few seconds later press flow light illuminated and flight attendants called on interphone to report smell along with haze/smoke in the cabin. Aircraft was not pressurizing. Declared emergency with ATC to return to ZZZ; accomplished QRH procedure; contacted dispatch; and briefed flight attendant. Haze/smoke in cabin dissipated. Normal overweight landing with arf standing by. Inspection by arf found no external damage. Taxied to gate and documented malfunction in aircraft logbook.

Title: AN MD80 DISPATCHED WITH A SINGLE PACK HAD SMOKE AND FUMES FROM THE REMAINING PACK ON TKOF. AN EMER WAS DECLARED WITH A RETURN TO LAND.

Narrative: DISPATCHED FROM ZZZ WITH SINGLE LEFT PACK; RIGHT PACK INOP DOCUMENTED WITH PLACARD IN LOGBOOK. TAXI OUT AND TKOF NORMAL. AT 2000 FT ENTERED CLOUD LAYER WITH ICING CONDITIONS; TURNED ON ENG AND AIRFOIL ANTI ICE SYSTEMS. FEW SECONDS LATER PRESS FLOW LIGHT ILLUMINATED AND FLT ATTENDANTS CALLED ON INTERPHONE TO REPORT SMELL ALONG WITH HAZE/SMOKE IN THE CABIN. ACFT WAS NOT PRESSURIZING. DECLARED EMER WITH ATC TO RETURN TO ZZZ; ACCOMPLISHED QRH PROC; CONTACTED DISPATCH; AND BRIEFED FLT ATTENDANT. HAZE/SMOKE IN CABIN DISSIPATED. NORMAL OVERWT LNDG WITH ARF STANDING BY. INSPECTION BY ARF FOUND NO EXTERNAL DAMAGE. TAXIED TO GATE AND DOCUMENTED MALFUNCTION IN ACFT LOGBOOK.
