Narrative:

Card calls for removing and replacing the center fuel tank scavenge check valve on the MD80. Inventory indicated no valves available. A mechanic removed the valve and associated tubing. I checked the condition of the valve and determined it to be serviceable. The mechanic reinstalled the valve and associated tubing. I signed the block for installing the valve signifying I accepted responsibility that the work had been satisfactorily completed. I inadvertently failed to print the mechanic's name that actually completed the work and enter the date. I immediately removed the logbook from the aircraft and retrieved the card. I met with my manager to discuss appropriate action. Phone calls were made to engineering explaining the fact that the existing check valve was deemed serviceable and reinstalled; in spite of the card calling for installation of a new valve; engineer revised the procedure to read; 'inspect and clean instead of install new valve.' I also spoke with quality assurance supervisor about this matter. I presented the amended page to the mechanic that installed the valve. He signed the block and the card was returned. In addition to the above stated actions to rectify the situation; I have reviewed gpm; section 09-03 signature requirements.

Title: AN MD80 JOB CARD REQUIRED REPLACEMENT OF CTR TANK SCAVENGE CHK VALVE. SUPVR ORDERED TECHNICIAN TO REMOVE; CLEAN; AND REINSTALL THE VALVE DUE TO PARTS NOT IN STOCK. TECHNICIAN INSTALLED VALVE BUT DID NOT SIGN FOR THE WORK.

I CHKED THE CONDITION OF THE VALVE AND DETERMINED IT TO BE SERVICEABLE. THE MECH REINSTALLED THE VALVE AND ASSOCIATED TUBING. I SIGNED THE BLOCK FOR INSTALLING THE VALVE SIGNIFYING I ACCEPTED RESPONSIBILITY THAT THE WORK HAD BEEN SATISFACTORILY COMPLETED. I INADVERTENTLY FAILED TO PRINT THE MECH'S NAME THAT ACTUALLY COMPLETED THE WORK AND ENTER THE DATE. I IMMEDIATELY REMOVED THE LOGBOOK FROM THE ACFT AND RETRIEVED THE CARD. I MET WITH MY MGR TO DISCUSS APPROPRIATE ACTION. PHONE CALLS WERE MADE TO ENGINEERING EXPLAINING THE FACT THAT THE EXISTING CHK VALVE WAS DEEMED SERVICEABLE AND REINSTALLED; IN SPITE OF THE CARD CALLING FOR INSTALLATION OF A NEW VALVE; ENGINEER REVISED THE PROC TO READ; 'INSPECT AND CLEAN INSTEAD OF INSTALL NEW VALVE.' IN ADDITION TO THE ABOVE STATED ACTIONS TO RECTIFY THE SIT; I HAVE REVIEWED GPM; SECTION 09-03 SIGNATURE REQUIREMENTS.
