Narrative:

Filed at FL280; assumed because of strong headwinds and an attempt by the 'default' flight plan to make an on-time arrival. Over the gulf of mexico turbulence became 'moderate' and I elected to climb to a higher altitude. At no time did I realize that the 'altitude alert' light 'inoperative' would restrict us from the higher altitudes. I just didn't realize that we were restr to lower; if we indeed were.

Title: B767-300 FLT CREW OPERATES IN RVSM AIRSPACE WITH INOP EQUIP.

Narrative: FILED AT FL280; ASSUMED BECAUSE OF STRONG HEADWINDS AND AN ATTEMPT BY THE 'DEFAULT' FLT PLAN TO MAKE AN ON-TIME ARR.
