Narrative:

Upon checking in with departure control and climbing through about 4500 ft we were instructed to level at 5000 ft (original clearance limit was 7000 ft). Once clear of the airbus ahead; we were cleared to bayta intersection to resume the maxxo 1 departure. The filed departure was the chezz 2; which shares bayta intersection. Both PF (captain) and I (first officer) had overlooked the revised departure requirement of our pre departure clearance received before pushback. Contributing factors to this oversight were distrs on the flight deck involving an incorrect cabin security check form and a maintenance question regarding the aircraft landing status. Once cleared to bayta; we reprogrammed the FMC and proceeded to fly the remainder of the maxxo 1 departure.

Title: B757-200 FLT CREW FAILS TO RECOGNIZE REVISED PORTION OF PDC DELIVERED RTE.

Narrative: UPON CHKING IN WITH DEP CTL AND CLBING THROUGH ABOUT 4500 FT WE WERE INSTRUCTED TO LEVEL AT 5000 FT (ORIGINAL CLRNC LIMIT WAS 7000 FT). CONTRIBUTING FACTORS TO THIS OVERSIGHT WERE DISTRS ON THE FLT DECK INVOLVING AN INCORRECT CABIN SECURITY CHK FORM AND A MAINT QUESTION REGARDING THE ACFT LNDG STATUS. ONCE CLRED TO BAYTA; WE REPROGRAMMED THE FMC AND PROCEEDED TO FLY THE REMAINDER OF THE MAXXO 1 DEP.
