Narrative:

Jetway moving away from aircraft ground ready for push we get permission from ramp control and ground asks for brake release. After lengthy pause push crew says jetway has not moved far enough away and we reset the brakes and the push crew went to assist with the jetbridge. Jetbridge started to move again and it impacted the left engine nacelle gouging a 3 inch hole in the top of the nacelle. As we sat waiting for the jetway to come back to offload our passenger from the now grounded aircraft; operation asked us to have ACARS send in the report so they could get control of the passenger manifest and move people to other flts. The only way to do this is to open a door. I asked purser to disarm the door and crack it open. Flight attendant had another flight attendant and one of our capts verify the door was disarmed; which it was. Flight attendant then opened the door and the in report was sent. Flight attendant then asked me if I wanted the door shut and I said yes. As flight attendant closed the door the handle gave some resistance and she pushed on it harder. At this point; the slide deployed and inflated (door 1L). 3 people independently verified that the door slide was disarmed before opening the door. Supplemental information from acn 709131: brakes released; cleared to push; tug driver said there was something wrong with the jetway. Reset brakes. The jetway moved and the lower part where the amber light is; contacted the top forward portion of the left engine nacelle causing damage. The jetway then moved away from the aircraft and maintenance came out to assess the damage. Damage to the engine was significant enough to ground the aircraft. Operations then requested an 'in' time to start the paperwork due to the incident. At that time; we were uncertain as to the length of the delay to get the jetway out to the airplane to open 2L and get our in-time as well as let the people off the aircraft. The captain asked the purser to crack 1L to get our in-time. She disarmed the door after confirming that it was disarmed with her flying partner and another deadheading captain. The door was opened normally. She then went to close the door and had difficulty pushing the handle; confirmed by the deadheading captain. At that point; with the door disarmed and attempts made to close it; the slide pack released and the slide at 1L deployed. At this point; the jetway was still back from the aircraft and there was no way to bring it to the airplane because the slide was in the way of the wheels. The slide was deflated and the jetway was brought back and the people were deplaned. Our flight was canceled.

Title: A B757-200 PARKED AT THE GATE IN SEA AND WAS STRUCK BY A JETWAY; DAMAGING A NACELLE. A FLT ATTENDANT THEN BLEW A SLIDE OPENING A DOOR.

Narrative: JETWAY MOVING AWAY FROM ACFT GND READY FOR PUSH WE GET PERMISSION FROM RAMP CTL AND GND ASKS FOR BRAKE RELEASE. AFTER LENGTHY PAUSE PUSH CREW SAYS JETWAY HAS NOT MOVED FAR ENOUGH AWAY AND WE RESET THE BRAKES AND THE PUSH CREW WENT TO ASSIST WITH THE JETBRIDGE. JETBRIDGE STARTED TO MOVE AGAIN AND IT IMPACTED THE L ENG NACELLE GOUGING A 3 INCH HOLE IN THE TOP OF THE NACELLE. AS WE SAT WAITING FOR THE JETWAY TO COME BACK TO OFFLOAD OUR PAX FROM THE NOW GNDED ACFT; OP ASKED US TO HAVE ACARS SEND IN THE RPT SO THEY COULD GET CTL OF THE PAX MANIFEST AND MOVE PEOPLE TO OTHER FLTS. THE ONLY WAY TO DO THIS IS TO OPEN A DOOR. I ASKED PURSER TO DISARM THE DOOR AND CRACK IT OPEN. FLT ATTENDANT HAD ANOTHER FLT ATTENDANT AND ONE OF OUR CAPTS VERIFY THE DOOR WAS DISARMED; WHICH IT WAS. FLT ATTENDANT THEN OPENED THE DOOR AND THE IN RPT WAS SENT. FLT ATTENDANT THEN ASKED ME IF I WANTED THE DOOR SHUT AND I SAID YES. AS FLT ATTENDANT CLOSED THE DOOR THE HANDLE GAVE SOME RESISTANCE AND SHE PUSHED ON IT HARDER. AT THIS POINT; THE SLIDE DEPLOYED AND INFLATED (DOOR 1L). 3 PEOPLE INDEPENDENTLY VERIFIED THAT THE DOOR SLIDE WAS DISARMED BEFORE OPENING THE DOOR. SUPPLEMENTAL INFO FROM ACN 709131: BRAKES RELEASED; CLRED TO PUSH; TUG DRIVER SAID THERE WAS SOMETHING WRONG WITH THE JETWAY. RESET BRAKES. THE JETWAY MOVED AND THE LOWER PART WHERE THE AMBER LIGHT IS; CONTACTED THE TOP FORWARD PORTION OF THE L ENG NACELLE CAUSING DAMAGE. THE JETWAY THEN MOVED AWAY FROM THE ACFT AND MAINT CAME OUT TO ASSESS THE DAMAGE. DAMAGE TO THE ENG WAS SIGNIFICANT ENOUGH TO GND THE ACFT. OPS THEN REQUESTED AN 'IN' TIME TO START THE PAPERWORK DUE TO THE INCIDENT. AT THAT TIME; WE WERE UNCERTAIN AS TO THE LENGTH OF THE DELAY TO GET THE JETWAY OUT TO THE AIRPLANE TO OPEN 2L AND GET OUR IN-TIME AS WELL AS LET THE PEOPLE OFF THE ACFT. THE CAPT ASKED THE PURSER TO CRACK 1L TO GET OUR IN-TIME. SHE DISARMED THE DOOR AFTER CONFIRMING THAT IT WAS DISARMED WITH HER FLYING PARTNER AND ANOTHER DEADHEADING CAPT. THE DOOR WAS OPENED NORMALLY. SHE THEN WENT TO CLOSE THE DOOR AND HAD DIFFICULTY PUSHING THE HANDLE; CONFIRMED BY THE DEADHEADING CAPT. AT THAT POINT; WITH THE DOOR DISARMED AND ATTEMPTS MADE TO CLOSE IT; THE SLIDE PACK RELEASED AND THE SLIDE AT 1L DEPLOYED. AT THIS POINT; THE JETWAY WAS STILL BACK FROM THE ACFT AND THERE WAS NO WAY TO BRING IT TO THE AIRPLANE BECAUSE THE SLIDE WAS IN THE WAY OF THE WHEELS. THE SLIDE WAS DEFLATED AND THE JETWAY WAS BROUGHT BACK AND THE PEOPLE WERE DEPLANED. OUR FLT WAS CANCELED.
