Narrative:

On climb out from okbk we received multiple heading and altitude changes and did not accomplish resetting the altimeter at transition altitude. This was also at approximately XA00 and 10+ hours into our duty day. We proceeded through iraq airspace and when crossing into euro rvsm ankara saw the altitude deviation of 400 ft. We immediate reset to 1013 millibars and descended to our correct level. 3 pilots watched this happen and none noticed it. I have to believe fatigue was a large contributor. I also missed the item on the en route climb checklist. Perhaps we should be doing the checklist at TA rather than FL180.

Title: MD11 FLT CREW HAS AN ENRTE ALTDEV.

Narrative: ON CLBOUT FROM OKBK WE RECEIVED MULTIPLE HDG AND ALT CHANGES AND DID NOT ACCOMPLISH RESETTING THE ALTIMETER AT TRANSITION ALT. THIS WAS ALSO AT APPROX XA00 AND 10+ HRS INTO OUR DUTY DAY. I ALSO MISSED THE ITEM ON THE ENRTE CLB CHKLIST. PERHAPS WE SHOULD BE DOING THE CHKLIST AT TA RATHER THAN FL180.
