Narrative:

On flight into sfo; I was responsible for this door; #1 and #2. We had a close connection. I was not aware that I did not disarm my doors. That's something that I am very conscious of; however; I apparently did not do so on this occasion. I truly apologize for this irresponsible act. I do realize the severity of this situation. For the past few days; I have wondered why or how I neglected disarming my door at 1L and 1R. Last night; I realized why and how. However; this does not negate the fact that I was negligent on my duties. Upon landing; one of the doors' carrier was not latched completely. The door and all its contents came out on landing. There was a rack of wine glasses and rock glasses stowed there. Needless to say; glass was everywhere. I immediately upon the plane coming to a complete stop at the gate; proceeded to clean up the glass. At the same time; a first class passenger who had items in the first class closet asked me to retrieve his belongings in the closet. I immediately continued to clean up the glass after helping him. We also had a close connection to another flight and the flight was a few mins late arriving. We had to rush to our connecting flight. With all the distrs; it did not occur to me that I had neglected to disarm my door. I realize that this is a serious offense and I accept full responsibility for my actions. I am overwhelmed that I did this. Supplemental information from acn 683553: 1L and 1R door in first class were not disarmed after announcement was made. I was flight attendant #4. I checked doors #3 and #4 and they were disarmed. As I went forward I observed that 2L was disarmed and 2R was disarmed. I was on my way to the first class compartment when either a passenger or a flight attendant interrupted me with a question. I was distraction and forgot to check whether 1L and 1R were disarmed. In the past I have found 2 doors armed and I disarmed them. This was before this procedure was in place. I would suggest that a checklist item be added to include a check off for disarming doors during the purser briefing. This policy of disarming doors has not been consistently followed. Including this action on the checklist would help it from being overlooked. Callback conversation with reporter revealed the following information: reporter stated that the process for disarming the aircraft doors involves 2 flight attendants; one has primary responsibility and a secondary flight attendant will check all doors disarmed. Both flight attendants in this case were distraction by events and did not check the 1L/1R doors. There were no slides deployed as a result of this performance lapse.

Title: AT GATE ARR A B757-200 FLT ATTENDANT FAILED TO DISARM THE ACFT'S L1-L2 DOORS BECAUSE OF BROKEN GLASS IN THE GALLEY.

Narrative: ON FLT INTO SFO; I WAS RESPONSIBLE FOR THIS DOOR; #1 AND #2. WE HAD A CLOSE CONNECTION. I WAS NOT AWARE THAT I DID NOT DISARM MY DOORS. THAT'S SOMETHING THAT I AM VERY CONSCIOUS OF; HOWEVER; I APPARENTLY DID NOT DO SO ON THIS OCCASION. I TRULY APOLOGIZE FOR THIS IRRESPONSIBLE ACT. I DO REALIZE THE SEVERITY OF THIS SITUATION. FOR THE PAST FEW DAYS; I HAVE WONDERED WHY OR HOW I NEGLECTED DISARMING MY DOOR AT 1L AND 1R. LAST NIGHT; I REALIZED WHY AND HOW. HOWEVER; THIS DOES NOT NEGATE THE FACT THAT I WAS NEGLIGENT ON MY DUTIES. UPON LNDG; ONE OF THE DOORS' CARRIER WAS NOT LATCHED COMPLETELY. THE DOOR AND ALL ITS CONTENTS CAME OUT ON LNDG. AT THE SAME TIME; A FIRST CLASS PAX WHO HAD ITEMS IN THE FIRST CLASS CLOSET ASKED ME TO RETRIEVE HIS BELONGINGS IN THE CLOSET. I IMMEDIATELY CONTINUED TO CLEAN UP THE GLASS AFTER HELPING HIM. WE ALSO HAD A CLOSE CONNECTION TO ANOTHER FLT AND THE FLT WAS A FEW MINS LATE ARRIVING. WE HAD TO RUSH TO OUR CONNECTING FLT. WITH ALL THE DISTRS; IT DID NOT OCCUR TO ME THAT I HAD NEGLECTED TO DISARM MY DOOR. I REALIZE THAT THIS IS A SERIOUS OFFENSE AND I ACCEPT FULL RESPONSIBILITY FOR MY ACTIONS. I AM OVERWHELMED THAT I DID THIS. SUPPLEMENTAL INFO FROM ACN 683553: 1L AND 1R DOOR IN FIRST CLASS WERE NOT DISARMED AFTER ANNOUNCEMENT WAS MADE. I WAS FLT ATTENDANT #4. I CHKED DOORS #3 AND #4 AND THEY WERE DISARMED. AS I WENT FORWARD I OBSERVED THAT 2L WAS DISARMED AND 2R WAS DISARMED. I WAS ON MY WAY TO THE FIRST CLASS COMPARTMENT WHEN EITHER A PAX OR A FLT ATTENDANT INTERRUPTED ME WITH A QUESTION. I WAS DISTR AND FORGOT TO CHK WHETHER 1L AND 1R WERE DISARMED. IN THE PAST I HAVE FOUND 2 DOORS ARMED AND I DISARMED THEM. THIS WAS BEFORE THIS PROC WAS IN PLACE. I WOULD SUGGEST THAT A CHKLIST ITEM BE ADDED TO INCLUDE A CHK OFF FOR DISARMING DOORS DURING THE PURSER BRIEFING. THIS POLICY OF DISARMING DOORS HAS NOT BEEN CONSISTENTLY FOLLOWED. INCLUDING THIS ACTION ON THE CHKLIST WOULD HELP IT FROM BEING OVERLOOKED. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATED THAT THE PROCESS FOR DISARMING THE ACFT DOORS INVOLVES 2 FLT ATTENDANTS; ONE HAS PRIMARY RESPONSIBILITY AND A SECONDARY FLT ATTENDANT WILL CHK ALL DOORS DISARMED. BOTH FLT ATTENDANTS IN THIS CASE WERE DISTR BY EVENTS AND DID NOT CHK THE 1L/1R DOORS. THERE WERE NO SLIDES DEPLOYED AS A RESULT OF THIS PERFORMANCE LAPSE.
