Narrative:

On nov/sat/05; I replaced a #3 main landing gear brake on aircraft X. I had sold my work accomplished to the inspector. I was getting ready to leave work because my spouse was in the hospital. So my lead assigned the a&P mechanic (mr X) to complete my job on the brakes. He was having trouble connecting 2 clamps for brake heat detection lines. So he insisted we disconnect the brake pin; so he could partially move the brake assembly to gain access for the clamps. I was leaving and I saw him struggle with the brake pin bolt and nut. So I assisted him. After we connected the clamps; we reinserted the pin; bolt and nut. So I told my partner that the nut and bolt needed to be torqued and to insert a cotter pin. I had told him the paperwork was all signed off; except tire installation. He said 'no problem; go to the hospital and take care of your family.' the next day I came to work and the lead gave me the same job card. The tire was installed and signed off by the inspector. I ended up doing the leak and operations check. The aircraft had left on nov/tue/05. Aircraft X landed and was informed by the tower that they saw sparks during the landing and taxi. The aircraft was inspected and found the #3 main landing gear brake assembly pin disconnected. I feel if I wasn't worried about my spouse in the hospital and not rushing to get to the hospital; that I would have wrote up on my non routine job card that mr X and myself disconnected the pin; bolt and nut on the brake assembly. After the inspector had bought off the installation of the brake assembly; mr X didn't document it on paper; also/or he didn't torque the bolt and nut and install the safety cotter pin. You need to visually inspect yourself before you sign off aircraft documents. And document all removed parts. If this would have been done; then the torque would have been visual by inspector -- and then he would have told the mechanic to install the safety cotter pin. Also; the inspector should have seen that the cotter pin was missing before he gave mr X the ok to install the #3 main landing gear tire.

SO I ASSISTED HIM. AFTER WE CONNECTED THE CLAMPS; WE REINSERTED THE PIN; BOLT AND NUT. SO I TOLD MY PARTNER THAT THE NUT AND BOLT NEEDED TO BE TORQUED AND TO INSERT A COTTER PIN. I HAD TOLD HIM THE PAPERWORK WAS ALL SIGNED OFF; EXCEPT TIRE INSTALLATION. HE SAID 'NO PROB; GO TO THE HOSPITAL AND TAKE CARE OF YOUR FAMILY.' THE NEXT DAY I CAME TO WORK AND THE LEAD GAVE ME THE SAME JOB CARD. THE TIRE WAS INSTALLED AND SIGNED OFF BY THE INSPECTOR. I ENDED UP DOING THE LEAK AND OPS CHK. THE ACFT HAD LEFT ON NOV/TUE/05. ACFT X LANDED AND WAS INFORMED BY THE TWR THAT THEY SAW SPARKS DURING THE LNDG AND TAXI. THE ACFT WAS INSPECTED AND FOUND THE #3 MAIN LNDG GEAR BRAKE ASSEMBLY PIN DISCONNECTED. I FEEL IF I WASN'T WORRIED ABOUT MY SPOUSE IN THE HOSPITAL AND NOT RUSHING TO GET TO THE HOSPITAL; THAT I WOULD HAVE WROTE UP ON MY NON ROUTINE JOB CARD THAT MR X AND MYSELF DISCONNECTED THE PIN; BOLT AND NUT ON THE BRAKE ASSEMBLY. AFTER THE INSPECTOR HAD BOUGHT OFF THE INSTALLATION OF THE BRAKE ASSEMBLY; MR X DIDN'T DOCUMENT IT ON PAPER; ALSO/OR HE DIDN'T TORQUE THE BOLT AND NUT AND INSTALL THE SAFETY COTTER PIN. YOU NEED TO VISUALLY INSPECT YOURSELF BEFORE YOU SIGN OFF ACFT DOCUMENTS. AND DOCUMENT ALL REMOVED PARTS. IF THIS WOULD HAVE BEEN DONE; THEN THE TORQUE WOULD HAVE BEEN VISUAL BY INSPECTOR -- AND THEN HE WOULD HAVE TOLD THE MECH TO INSTALL THE SAFETY COTTER PIN. ALSO; THE INSPECTOR SHOULD HAVE SEEN THAT THE COTTER PIN WAS MISSING BEFORE HE GAVE MR X THE OK TO INSTALL THE #3 MAIN LNDG GEAR TIRE.
