Narrative:

Aircraft X climbed to 3000 ft. I thought I climbed aircraft X to 5000 ft. I wrote 5000 ft on the strip. I had mistakenly said '15000 ft' to aircraft X. I called traffic that was 2 O'clock position; 10 mi; level at 6000 ft. Aircraft X reported the traffic in sight when he was around 4000 ft. When I observed aircraft X leaving 5300 ft; I told him to descend and maintain 5000 ft.

Narrative: ACFT X CLBED TO 3000 FT. I THOUGHT I CLBED ACFT X TO 5000 FT. I WROTE 5000 FT ON THE STRIP. I HAD MISTAKENLY SAID '15000 FT' TO ACFT X.
