Narrative:

I was acting in the capacity of lead technician. I had assigned myself to work air carrier X; aircraft X with both mr X and mr Y. I don't recall what specific portion of the overnight/ETOPS check I had been working on that date. Upon the completion of my assignment; I went over to assist both mr X and mr Y who were replacing the left nosewheel assembly; due to being worn to limits. My involvement in this maintenance included loosening the valve core of the removed tire and installing the required shipping covers. I then rolled the unserviceable wheel to our shipping area and marked the exterior with serial number information. Later on in the evening; the air carrier representative passed me both the job instruction card and aircraft logbook requesting a confirmation signature. I signed all applicable confirmation boxes as requested. I was later made aware that the required exterior spacer (tang washer) was inadvertently omitted during installation. Confirmation of the installation of this washer was signed by me on both air carrier's job instruction and aircraft log. Callback conversation with reporter revealed the following information: the reporter stated the wheel and tire assembly was replaced and the job card block calling out the spacer installation had been marked 'north/a;' indicating the axle was externally threaded and did not require a spacer. The spacer was found later that day in the grease on the removed tire and wheel assembly.

Title: A B767-300 WAS DISPATCHED WITH THE L TIRE AND WHEEL ASSEMBLY MISSING THE REQUIRED SPACER.

I WAS LATER MADE AWARE THAT THE REQUIRED EXTERIOR SPACER (TANG WASHER) WAS INADVERTENTLY OMITTED DURING INSTALLATION. CONFIRMATION OF THE INSTALLATION OF THIS WASHER WAS SIGNED BY ME ON BOTH ACR'S JOB INSTRUCTION AND ACFT LOG. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE WHEEL AND TIRE ASSEMBLY WAS REPLACED AND THE JOB CARD BLOCK CALLING OUT THE SPACER INSTALLATION HAD BEEN MARKED 'N/A;' INDICATING THE AXLE WAS EXTERNALLY THREADED AND DID NOT REQUIRE A SPACER. THE SPACER WAS FOUND LATER THAT DAY IN THE GREASE ON THE REMOVED TIRE AND WHEEL ASSEMBLY.
