Narrative:

Taxied from FBO to active runway. Cleared for takeoff. Applied full power. At rotation speed; applied back yoke pressure. Nosewheel lifted off ground; right main wheel collapsed; followed by left then nose. Plane traveled approximately 1000 ft down runway before collapse. Plane skidded additional 800 ft. Cause of collapse is being checked by mechanics. Callback conversation with reporter revealed the following information: the reporter stated the preflight check was accomplished; but failed to note that the 3 green landing gear position indicating lights were not illuminated. Control lever was not locked in down detent. The aircraft incurred damage to the propellers; engine; and belly skin.

Narrative: TAXIED FROM FBO TO ACTIVE RWY. CLRED FOR TKOF. APPLIED FULL PWR. AT ROTATION SPD; APPLIED BACK YOKE PRESSURE. NOSEWHEEL LIFTED OFF GND; R MAIN WHEEL COLLAPSED; FOLLOWED BY L THEN NOSE. PLANE TRAVELED APPROX 1000 FT DOWN RWY BEFORE COLLAPSE. PLANE SKIDDED ADDITIONAL 800 FT. CAUSE OF COLLAPSE IS BEING CHKED BY MECHS. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE PREFLT CHK WAS ACCOMPLISHED; BUT FAILED TO NOTE THAT THE 3 GREEN LNDG GEAR POS INDICATING LIGHTS WERE NOT ILLUMINATED. CTL LEVER WAS NOT LOCKED IN DOWN DETENT. THE ACFT INCURRED DAMAGE TO THE PROPS; ENG; AND BELLY SKIN.
