Narrative:

I am filing this report due to me feeling mentally and physically unable to fly. I flew a 2-DAY trip with 12 hours 9 mins actual flying, which returned in oakland at XA00 on nov/mon/04. That evening I was in bed by XB30-XC00. 2 hours later I was awakened by the crew desk at XE11 for an assignment that day at XQ00. Needless to say, I got very little sleep that night. I then deadheaded to lax that day to a layover with an XG00 report time the following morning. That day, which was nov/wed, I had a duty day of 11 hours. By the end of the day I was mentally and physically not feeling well. I was scheduled to fly another XG45 trip the next morning but called in sick. I had 94 hours 54 mins credit hours the prior month and maximum'ed out 2 days early.
