Narrative:
It was brought to my attention by station chairman that the loi (FAA letter of investigation) I received from FAA was for a different aircraft than my previous as soon as possible report and I want to report the event is same. When I sign off the back check on ZZZ card I 'north/a' (not applicable-task not applicable to model aircraft) section 1A, 7, 8, 9-7 and 8 should be 'north/a' and 9 should be in the next section box for signoff to be correct.

Title: A B757-200 SVC CHK JOB CARD HAS A CONFUSING FORMAT. EACH TASK ON THE CARD IS NUMBERED 1, 2, 3, ETC. A SIGNOFF BOX FOR INITIALS IS LOCATED TO R IN A VERT COLUMN. THE SIGNOFF BOXES DO NOT LINE UP WITH THE TASKS.
Narrative: IT WAS BROUGHT TO MY ATTN BY STATION CHAIRMAN THAT THE LOI (FAA LETTER OF INVESTIGATION) I RECEIVED FROM FAA WAS FOR A DIFFERENT ACFT THAN MY PREVIOUS ASAP RPT AND I WANT TO RPT THE EVENT IS SAME. WHEN I SIGN OFF THE BACK CHK ON ZZZ CARD I 'N/A' (NOT APPLICABLE-TASK NOT APPLICABLE TO MODEL ACFT) SECTION 1A, 7, 8, 9-7 AND 8 SHOULD BE 'N/A' AND 9 SHOULD BE IN THE NEXT SECTION BOX FOR SIGNOFF TO BE CORRECT.
