Narrative:

I was just accepting the control position from the previous controller. I had heard the previous controller instruct the BE55 'no delay down to 3000 ft.' I attempted to hand off a third aircraft that was unrelated to this incident to an adjacent facility. I then looked at the BE55 and the SF34 and saw that they were on converging course both at 4000 ft. I issued turns to both aircraft. The SF34 reported the BE55 in sight and I instructed him to 'maintain visual separation.' separation was already lost.
