Narrative:

While taxiing for departure runway 4R lft, we were switched to tower and told to contact them for departure. We were hurrying to meet a flow control release time. As we started to taxi past the hold short line into position, we realized that we hadn't been cleared into position yet, and at the same time tower advised us that we hadn't been cleared. We apologized and they said it was ok (early morning, no traffic) and cleared us for takeoff. Rush to taxi out to meet flow control time caused inattn to detail. We misunderstood switch to tower as signal to continue out and call them ready for departure. Fatigued slightly and not thinking clearly.
