Narrative:
On takeoff/climb out, a shredded tire tread disabled hydraulics by damaging hydraulic lines above #3 main tire. On return to land, hydraulic system 'a' failed. Hydraulic system 'B' lost #1 pump and was losing fluid quantity. After second pass by tower for a landing gear check, an overweight landing was accomplished. Aircraft shutdown and towed to gate. Shredded tire and metal debris recovered from runway after our departure. Suspect 2 unidented metal chunks caused #3 main tire to fail on takeoff roll.

Title: A B737-300 FO RPTED THAT FOD CAUSED THE #3 MAIN TIRE TO SHRED DURING TKOF FROM SLC. A RETURN TO LAND WAS SUCCESSFULLY ACCOMPLISHED AS THE SHREDDED TIRE DAMAGED THE HYD SYS, CAUSING FLUID LEAKAGE.
Narrative: ON TKOF/CLBOUT, A SHREDDED TIRE TREAD DISABLED HYDS BY DAMAGING HYD LINES ABOVE #3 MAIN TIRE. ON RETURN TO LAND, HYD SYS 'A' FAILED. HYD SYS 'B' LOST #1 PUMP AND WAS LOSING FLUID QUANTITY. AFTER SECOND PASS BY TWR FOR A LNDG GEAR CHK, AN OVERWT LNDG WAS ACCOMPLISHED. ACFT SHUTDOWN AND TOWED TO GATE. SHREDDED TIRE AND METAL DEBRIS RECOVERED FROM RWY AFTER OUR DEP. SUSPECT 2 UNIDENTED METAL CHUNKS CAUSED #3 MAIN TIRE TO FAIL ON TKOF ROLL.
