Narrative:
This report encompasses the last 8 days. I have had 1 day off. In the remaining 7 days I have worked 38.5 hours of which 30 hours is flight time and 8.5 hours is deadhead. I have been reassigned a total of 6 times and have been scheduled over the monthly limit of 95 hours. I am fatigued and feel that there is a sufficient risk to any future flts this week that I have asked the flight office in ZZZ to take me off the reserve schedule for my final day of reserve.
