Narrative:

During approach to ord runway 4R, we selected flaps 1 degree at 220 KIAS. Leading edge flaps transit light was on with the trailing edge flaps stuck between the up position and flaps 1 degree. Referring to flight manual, we followed the flow chart and applied the trailing edge flaps disagree procedure. Using alternate flap extension procedures, we selected flaps 15 degrees and had normal leading edge flaps and slat indications. We declared an emergency and landed uneventfully with flaps 15 degrees.
