Narrative:

Having not flown for several weeks (vacation), there were 3 revisions in my mail box when I reported for work. I completed all changes before flight. During cruise, I noticed that we were filed for the 'UKW7' arrival into dfw, but my pubs were the 'UKW8.' this was part of one of the changes I had made prior to flight. I noticed the date on the page was dec/fri/03. But there is a small black box that says 'effective through dec/thu/03.' because we didn't have a current arrival page, center and approach control were forced to provide vectors for the arrival. I obviously should have noticed the effective data but it would have helped if the envelope the change came in had had a large note stating its effective date.
