Narrative:

Air carrier X and air carrier Y were both on frequency. Radar controller issued climb to air carrier Y with head-on traffic 15-20 mi out. I questioned the clearance with radar controller. He believed that he had issued climb to air carrier X, not air carrier Y. I said that I believed he had issued clearance to air carrier Y not air carrier X. He believed that he hadn't and issued further climb to air carrier Y. I again said he had climbed the wrong aircraft. After air carrier Y had already passed FL290 climbing, he issued a 'maintain FL290' clearance. Clearance was not in time to prevent error.

Title: ZOB ASSOCIATED INSTRUCTOR ALERTS RADAR CTLR OF WRONG ACR CLB CLRNC. RADAR CTLR QUESTIONS ASSOCIATE INSTRUCTOR, REALIZING THE CONFLICT TOO LATE TO MAINTAIN SEPARATION.

Narrative: ACR X AND ACR Y WERE BOTH ON FREQ. RADAR CTLR ISSUED CLB TO ACR Y WITH HEAD-ON TFC 15-20 MI OUT. I QUESTIONED THE CLRNC WITH RADAR CTLR. HE BELIEVED THAT HE HAD ISSUED CLB TO ACR X, NOT ACR Y. I SAID THAT I BELIEVED HE HAD ISSUED CLRNC TO ACR Y NOT ACR X. HE BELIEVED THAT HE HADN'T AND ISSUED FURTHER CLB TO ACR Y. I AGAIN SAID HE HAD CLBED THE WRONG ACFT. AFTER ACR Y HAD ALREADY PASSED FL290 CLBING, HE ISSUED A 'MAINTAIN FL290' CLRNC. CLRNC WAS NOT IN TIME TO PREVENT ERROR.
