Narrative:

On the night of jan/tue/03, I was working late night shift. I was assigned to work the #2 engine package, which includes opening and closing of #2 engine pylon panels. I closed the upper pylon panels which included the panel that caused the event. To the best of my knowledge when I closed the panel it was secure, all indicators were in the locked position. I am not aware of, nor did I see, anyone re-open the panel, but I wasn't really paying attention to that area. I feel it is likely that someone opened the panel after I had closed and secured the panel.

Title: A B737-300 WAS RETURNED TO SVC AFTER A 'B' SVC CHK WITH A #2 ENG PYLON NOT SECURED. DISCOVERED AND SECURED PRIOR TO FLT.

Narrative: ON THE NIGHT OF JAN/TUE/03, I WAS WORKING LATE NIGHT SHIFT. I WAS ASSIGNED TO WORK THE #2 ENG PACKAGE, WHICH INCLUDES OPENING AND CLOSING OF #2 ENG PYLON PANELS. I CLOSED THE UPPER PYLON PANELS WHICH INCLUDED THE PANEL THAT CAUSED THE EVENT. TO THE BEST OF MY KNOWLEDGE WHEN I CLOSED THE PANEL IT WAS SECURE, ALL INDICATORS WERE IN THE LOCKED POS. I AM NOT AWARE OF, NOR DID I SEE, ANYONE RE-OPEN THE PANEL, BUT I WASN'T REALLY PAYING ATTN TO THAT AREA. I FEEL IT IS LIKELY THAT SOMEONE OPENED THE PANEL AFTER I HAD CLOSED AND SECURED THE PANEL.
