Narrative:

We arrived in mtpp about 30 mins late due to maintenance problems in ZZZ. Maintenance had placarded our #2 bleed air system and air conditioning pack inoperative. This resulted in an altitude restr of FL310. After landing mtpp, we went to operations for the new release. The agent had already pulled the paperwork. I reviewed the route of flight and the fuel load as well as the alternate requirements. Everything seemed normal since I had just flown the same aircraft and route on the inbound leg. I checked the bottom of the flight plan and noted the MEL items were properly annotated and then signed the release. After engine start, as we were taxiing, we received an ACARS message from dispatch stating restr altitude to FL310. I was aware of this from the inbound leg and thought nothing of it. As we back taxied down the runway, we received a revised release with a corrected fuel burn. The changes were easily accommodated in our current fuel load so we pressed on. The reason I am writing this report is because this was a very insidious problem that was set up by dispatch's failure to apply the necessary restrs and my failure to make sure that these corrections were applied to the fuel calculations. Dispatch caught it before takeoff, but this sure could have been ugly given the right set of circumstances. You can chalk it up to any number of distractions we have to deal with every day, or the length of our duty day, combined with back to back early morning getups.

Title: B733 DEPARTED GATE WITH DISPATCH RELEASE NOT REFLECTING REQUIRED LIMITATION RESULTING FROM THE MAINT STATUS OF THE ACFT. AMENDED RELEASE PRIOR TO TKOF.

EVERYTHING SEEMED NORMAL SINCE I HAD JUST FLOWN THE SAME ACFT AND RTE ON THE INBOUND LEG. THE REASON I AM WRITING THIS RPT IS BECAUSE THIS WAS A VERY INSIDIOUS PROB THAT WAS SET UP BY DISPATCH'S FAILURE TO APPLY THE NECESSARY RESTRS AND MY FAILURE TO MAKE SURE THAT THESE CORRECTIONS WERE APPLIED TO THE FUEL CALCULATIONS. DISPATCH CAUGHT IT BEFORE TKOF, BUT THIS SURE COULD HAVE BEEN UGLY GIVEN THE RIGHT SET OF CIRCUMSTANCES. YOU CAN CHALK IT UP TO ANY NUMBER OF DISTRACTIONS WE HAVE TO DEAL WITH EVERY DAY, OR THE LENGTH OF OUR DUTY DAY, COMBINED WITH BACK TO BACK EARLY MORNING GETUPS.
