Narrative:

While at the gate in hpn, I filled out an incomplete weight and balance. We were flying out empty, no passenger or baggage, and I somehow forgot to add the fuel loaded and to subtract the fuel burn. I caught this on taxi out and had it complete before takeoff. But the copy I turned in was incomplete. I was tired, a little distraction, and this was my 5TH day back in the right seat after an industry downgrade.
