Narrative:

On jul/mon/02, worked as an inspector assigned left wing on aircraft. On jul/sun O2, panel 33501 (#4 canoe fairing) separated from aircraft -- found dangling upon landing in ZZZ. I was the inspector responsible for the buy-off of all panels removed on left wing for 'C' check. I verified all panels were properly installed, including panel 3501 which requires a safety (cotter pin). As a routine, I always tap on #4 and #5 canoe fairings, even after visually checking all bolts installed and safetied. Panel #3501 was installed properly and safetied when it left 'C' check back on jul/mon/02. This incident occurred approximately 2 weeks after my inspection. I know for a fact panel 3501 was installed properly.

Title: A B737-300 WAS FOUND TO HAVE #4 FAIRING HANGING FROM THE WING. FAIRING WAS INSTALLED AND INSPECTED 14 DAYS PRIOR TO SEPARATION.

Narrative: ON JUL/MON/02, WORKED AS AN INSPECTOR ASSIGNED L WING ON ACFT. ON JUL/SUN O2, PANEL 33501 (#4 CANOE FAIRING) SEPARATED FROM ACFT -- FOUND DANGLING UPON LNDG IN ZZZ. I WAS THE INSPECTOR RESPONSIBLE FOR THE BUY-OFF OF ALL PANELS REMOVED ON L WING FOR 'C' CHK. I VERIFIED ALL PANELS WERE PROPERLY INSTALLED, INCLUDING PANEL 3501 WHICH REQUIRES A SAFETY (COTTER PIN). AS A ROUTINE, I ALWAYS TAP ON #4 AND #5 CANOE FAIRINGS, EVEN AFTER VISUALLY CHKING ALL BOLTS INSTALLED AND SAFETIED. PANEL #3501 WAS INSTALLED PROPERLY AND SAFETIED WHEN IT LEFT 'C' CHK BACK ON JUL/MON/02. THIS INCIDENT OCCURRED APPROX 2 WKS AFTER MY INSPECTION. I KNOW FOR A FACT PANEL 3501 WAS INSTALLED PROPERLY.
