Narrative:

On jul/mon/02 I was assigned a preflight service check and a problem with the aircraft fishtailing on aircraft. After I accomplished my work on the preflight service check on aircraft, I proceeded to work on the problem of the aircraft fishtailing. I was informed by my technical crew chief to change both left and right servo-valve bungees. I changed the r-hand servo-valve bungee according to maintenance manual procedures. I informed management that the aircraft needed a functional check flight due to the fact both servos had been changed as a precautionary measure. I wrote up my paperwork. At no time was there an operations check done to see if that fixed the problem of the aircraft fishtailing. I also wrote up item #3 that the aircraft needed a functional check flight per gpm to determine a positive fix. Management signed it off that the aircraft did not need a functional check flight per gpm 10-01- F6. They also printed in the logbook that the left and right servo-valve bungees were changed and fixed the problem. They printed my name and employee number to the logbook page. At no time did I sign any paperwork showing that the plane was fixed. Callback conversation with reporter revealed the following information: the reporter stated the general procedures manual called for a flight test after replacement of flight control rigging or replacement of actuating units. The reporter said management interpreted the general procedures manual much differently and put the airplane in revenue service without the test flight. The reporter stated the airplane flew normally and the servo-vale bungee actuators fixed the problem.

Title: A FOKKER 100 WAS DISPATCHED IN NON COMPLIANCE WITH A REQUIRED FLT TEST NOT ACCOMPLISHED AFTER THE L AND R AILERON SERVO VALVE BUNGEES WERE REPLACED.

Narrative: ON JUL/MON/02 I WAS ASSIGNED A PREFLT SVC CHK AND A PROB WITH THE ACFT FISHTAILING ON ACFT. AFTER I ACCOMPLISHED MY WORK ON THE PREFLT SVC CHK ON ACFT, I PROCEEDED TO WORK ON THE PROB OF THE ACFT FISHTAILING. I WAS INFORMED BY MY TECHNICAL CREW CHIEF TO CHANGE BOTH L AND R SERVO-VALVE BUNGEES. I CHANGED THE R-HAND SERVO-VALVE BUNGEE ACCORDING TO MAINT MANUAL PROCS. AT NO TIME WAS THERE AN OPS CHK DONE TO SEE IF THAT FIXED THE PROB OF THE ACFT FISHTAILING. I ALSO WROTE UP ITEM #3 THAT THE ACFT NEEDED A FUNCTIONAL CHK FLT PER GPM TO DETERMINE A POSITIVE FIX. MGMNT SIGNED IT OFF THAT THE ACFT DID NOT NEED A FUNCTIONAL CHK FLT PER GPM 10-01- F6. THEY ALSO PRINTED IN THE LOGBOOK THAT THE L AND R SERVO-VALVE BUNGEES WERE CHANGED AND FIXED THE PROB. THEY PRINTED MY NAME AND EMPLOYEE NUMBER TO THE LOGBOOK PAGE. AT NO TIME DID I SIGN ANY PAPERWORK SHOWING THAT THE PLANE WAS FIXED. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE GENERAL PROCS MANUAL CALLED FOR A FLT TEST AFTER REPLACEMENT OF FLT CTL RIGGING OR REPLACEMENT OF ACTUATING UNITS. THE RPTR SAID MGMNT INTERPED THE GENERAL PROCS MANUAL MUCH DIFFERENTLY AND PUT THE AIRPLANE IN REVENUE SVC WITHOUT THE TEST FLT. THE RPTR STATED THE AIRPLANE FLEW NORMALLY AND THE SERVO-VALE BUNGEE ACTUATORS FIXED THE PROB.
