Narrative:

At about 20 DME, approach control cleared us to 8000 ft and left 15 degrees (230 degree heading). At approximately 1 DME, cleared to 7000 ft and asked if field in sight for visual approach. At approximately 12 DME we accepted visual approach and turned toward 5 mi centerline fix and began descent to cross at 4500 ft, 140 KTS. Egpws terrain display showed low density on our flight path and airport and vicinity clearly in view. We also appeared to be close to extended runway centerline (runway 28R) when cleared for visual approach at 7000 ft. At about 10 DME, egpws 'terrain, pull up' warning. Immediately applied toga power and climbed. Warning ceased promptly. In retrospect and close analysis, we may have been slightly north of centerline near 5000 ft terrain spots when turn and descent initiated and descent rate too high initially. ATC terrain alert given by approach control simultaneous with egpws.

IN RETROSPECT AND CLOSE ANALYSIS, WE MAY HAVE BEEN SLIGHTLY N OF CTRLINE NEAR 5000 FT TERRAIN SPOTS WHEN TURN AND DSCNT INITIATED AND DSCNT RATE TOO HIGH INITIALLY. ATC TERRAIN ALERT GIVEN BY APCH CTL SIMULTANEOUS WITH EGPWS.
