Narrative:

On approach to phx, we were with the initial approach controller and were told to expect runway 26. We were then handed off to the final approach controller and were advised of traffic at 10 O'clock position. We verified we had the traffic in sight and were told to follow the traffic, cleared for the visual approach. That traffic proceeded to runway 26. As we turned final for runway 26, we were advised that we were overshooting the runway. I advised the controller that we were cleared for the visual runway 26. He stated that we were supposed to be on runway 25L. I'm not sure if I and my first officer missed the runway 25L in the initial clearance, or if we were just cleared for the visual approach and there was a misunderstanding between the controllers. It is my standard practice to repeat back any runways in a clearance, but this time I distinctly remember saying simply 'cleared for the visual approach.' supplemental information from acn 546429: I was now on a 1-DAY trip to finish 4 consecutive days and was scheduled for 13 hours of duty with an XA30 show. It was the last leg of a 4-LEG day and I was definitely not 100%. At the designated position we were handed off to a second phx approach frequency. One of the controllers told us to expect runway 26. After the handoff, the second controller pointed out our arrival traffic and cleared us for the visual approach to what I thought was runway 26. About 8 seconds after lining up on runway 26, the controller pointed out traffic to the north runway and mentioned that we appeared to be slightly through the course. The controller said he thought he had cleared us for a visual to runway 25L and asked why we were lined up for runway 26. My captain explained we had been told to expect runway 26. We were reclred for a visual to runway 25L and landed without further incident. I was definitely fatigued and was concentrated on the aircraft more than the radio. I didn't even hear what runway we were cleared for, I just lined up on the runway we were told to expect. Fatigue was a huge factor in my lining up on the wrong runway.

Title: FINAL APCH CTLR ALLEGES APCH TO WRONG RWY 26 BY A CL65 FLC DURING A VISUAL APCH TO PHX, AZ.

Narrative: ON APCH TO PHX, WE WERE WITH THE INITIAL APCH CTLR AND WERE TOLD TO EXPECT RWY 26. WE WERE THEN HANDED OFF TO THE FINAL APCH CTLR AND WERE ADVISED OF TFC AT 10 O'CLOCK POS. WE VERIFIED WE HAD THE TFC IN SIGHT AND WERE TOLD TO FOLLOW THE TFC, CLRED FOR THE VISUAL APCH. THAT TFC PROCEEDED TO RWY 26. AS WE TURNED FINAL FOR RWY 26, WE WERE ADVISED THAT WE WERE OVERSHOOTING THE RWY. I ADVISED THE CTLR THAT WE WERE CLRED FOR THE VISUAL RWY 26. HE STATED THAT WE WERE SUPPOSED TO BE ON RWY 25L. I'M NOT SURE IF I AND MY FO MISSED THE RWY 25L IN THE INITIAL CLRNC, OR IF WE WERE JUST CLRED FOR THE VISUAL APCH AND THERE WAS A MISUNDERSTANDING BTWN THE CTLRS. IT IS MY STANDARD PRACTICE TO REPEAT BACK ANY RWYS IN A CLRNC, BUT THIS TIME I DISTINCTLY REMEMBER SAYING SIMPLY 'CLRED FOR THE VISUAL APCH.' SUPPLEMENTAL INFO FROM ACN 546429: I WAS NOW ON A 1-DAY TRIP TO FINISH 4 CONSECUTIVE DAYS AND WAS SCHEDULED FOR 13 HRS OF DUTY WITH AN XA30 SHOW. AFTER THE HDOF, THE SECOND CTLR POINTED OUT OUR ARR TFC AND CLRED US FOR THE VISUAL APCH TO WHAT I THOUGHT WAS RWY 26. ABOUT 8 SECONDS AFTER LINING UP ON RWY 26, THE CTLR POINTED OUT TFC TO THE N RWY AND MENTIONED THAT WE APPEARED TO BE SLIGHTLY THROUGH THE COURSE. THE CTLR SAID HE THOUGHT HE HAD CLRED US FOR A VISUAL TO RWY 25L AND ASKED WHY WE WERE LINED UP FOR RWY 26. MY CAPT EXPLAINED WE HAD BEEN TOLD TO EXPECT RWY 26. WE WERE RECLRED FOR A VISUAL TO RWY 25L AND LANDED WITHOUT FURTHER INCIDENT. I WAS DEFINITELY FATIGUED AND WAS CONCENTRATED ON THE ACFT MORE THAN THE RADIO. I DIDN'T EVEN HEAR WHAT RWY WE WERE CLRED FOR, I JUST LINED UP ON THE RWY WE WERE TOLD TO EXPECT. FATIGUE WAS A HUGE FACTOR IN MY LINING UP ON THE WRONG RWY.
