Narrative:

On the night of apr/tue/02, I performed a second stage turbine wheel borescope inspection. The inspection went well, no defects noted. The mechanic I was working with began to reinstall the engine's borescope port plugs. He installed the inner plug and I observed him torque the attach screws. He then installed the containment plate and I watched him torque its attaching hardware. At this point we were interrupted by another employee. The mechanic continued to install the remaining 2 covers. I observed the torque of the final outer case cover. We both signed off the work card. In the morning, after third shift completed their 1A check on the aircraft, the aircraft was pushed outside for engine runs and leak checks. Upon starting the right hand engine, the inner fan duct borescope cover exited the engine. A logbook write-up was made for aircraft X. Item #1 'right-hand engine second stage turbine blade borescope inner fan duct cover requires reinstallation.'

Title: A MD82 AFTER COMPLETION OF #1 ENG BORESCOPE THE ENG WAS RUN FOR LEAK CHKS AND WHILE RUNNING THE INNER FAN DUCT BORESCOPE COVER DEPARTED THE ENG.

Narrative: ON THE NIGHT OF APR/TUE/02, I PERFORMED A SECOND STAGE TURBINE WHEEL BORESCOPE INSPECTION. THE INSPECTION WENT WELL, NO DEFECTS NOTED. THE MECH I WAS WORKING WITH BEGAN TO REINSTALL THE ENG'S BORESCOPE PORT PLUGS. HE INSTALLED THE INNER PLUG AND I OBSERVED HIM TORQUE THE ATTACH SCREWS. HE THEN INSTALLED THE CONTAINMENT PLATE AND I WATCHED HIM TORQUE ITS ATTACHING HARDWARE. AT THIS POINT WE WERE INTERRUPTED BY ANOTHER EMPLOYEE. THE MECH CONTINUED TO INSTALL THE REMAINING 2 COVERS. I OBSERVED THE TORQUE OF THE FINAL OUTER CASE COVER. WE BOTH SIGNED OFF THE WORK CARD. IN THE MORNING, AFTER THIRD SHIFT COMPLETED THEIR 1A CHK ON THE ACFT, THE ACFT WAS PUSHED OUTSIDE FOR ENG RUNS AND LEAK CHKS. UPON STARTING THE RIGHT HAND ENG, THE INNER FAN DUCT BORESCOPE COVER EXITED THE ENG. A LOGBOOK WRITE-UP WAS MADE FOR ACFT X. ITEM #1 'R-HAND ENG SECOND STAGE TURBINE BLADE BORESCOPE INNER FAN DUCT COVER REQUIRES REINSTALLATION.'
