Narrative:

Upon walkaround of aircraft X, I found #4 tire had chords showing. I proceeded to print up maintenance manual paperwork. After collecting all tools and maintenance manual requirements, I began to remove the tire. It was raining and windy. There was a large puddle under the #4 tire. When I removed the nut, the spacer must have been stuck on the tire and when I removed the tire the spacer must have fallen into the puddle. Because the APU was running and I had on a hood and earmuffs, I didn't hear the spacer fall. The lighting was very poor thereby making it hard to see the spacer in the puddle.

Title: A B737-300 WAS DISPATCHED IN NON COMPLIANCE WITH #4 MAIN WHEEL AND TIRE ASSEMBLY INSTALLED WITH THE WHEEL SPACER MISSING.

Narrative: UPON WALKAROUND OF ACFT X, I FOUND #4 TIRE HAD CHORDS SHOWING. I PROCEEDED TO PRINT UP MAINT MANUAL PAPERWORK. AFTER COLLECTING ALL TOOLS AND MAINT MANUAL REQUIREMENTS, I BEGAN TO REMOVE THE TIRE. IT WAS RAINING AND WINDY. THERE WAS A LARGE PUDDLE UNDER THE #4 TIRE. WHEN I REMOVED THE NUT, THE SPACER MUST HAVE BEEN STUCK ON THE TIRE AND WHEN I REMOVED THE TIRE THE SPACER MUST HAVE FALLEN INTO THE PUDDLE. BECAUSE THE APU WAS RUNNING AND I HAD ON A HOOD AND EARMUFFS, I DIDN'T HEAR THE SPACER FALL. THE LIGHTING WAS VERY POOR THEREBY MAKING IT HARD TO SEE THE SPACER IN THE PUDDLE.
