Narrative:

The arrival ATIS for dulles stated visual approachs runways 19L/19R/12, single ground 121.9. The first officer flew a visual approach to runway 12. The aircraft landed normally and exited runway 12 at taxiway Q4. Normally, the tower instructs aircraft to remain with tower, however, there was no such instruction this time. Once past the hold lines, the captain continued taxi, after trying to contact tower. The radio was switched to the ground frequency 121.9. Ground control was advised of aircraft position on taxiway Q4. Ground 121.9 advised us to switch to 132.45. The captain kept taxiing without clearance to the gate. By this time the aircraft was passing the runway 19R approach hold lines. Ground 132.45 instructed the aircraft to 'hurry up across approach area runway 1L.' there was no conflict with arriving or departing aircraft. I believe this incident was caused by an inaccurate ATIS, single ground 121.9, tower not issuing instructions to remain with them and hold at taxiway Q4, the captain's failure to stop taxi at taxiway Q4 hold lines until taxi clearance was issued. Ground 132.45 should have held aircraft at taxiway Q. Poor communications by ATC (dulles tower) caused the first link in the sequence of miscoms that caused aircraft to pass runway 1L approach hold line.

NORMALLY, THE TWR INSTRUCTS ACFT TO REMAIN WITH TWR, HOWEVER, THERE WAS NO SUCH INSTRUCTION THIS TIME. ONCE PAST THE HOLD LINES, THE CAPT CONTINUED TAXI, AFTER TRYING TO CONTACT TWR. THE RADIO WAS SWITCHED TO THE GND FREQ 121.9. GND CTL WAS ADVISED OF ACFT POS ON TXWY Q4. GND 121.9 ADVISED US TO SWITCH TO 132.45. THE CAPT KEPT TAXIING WITHOUT CLRNC TO THE GATE. BY THIS TIME THE ACFT WAS PASSING THE RWY 19R APCH HOLD LINES. GND 132.45 INSTRUCTED THE ACFT TO 'HURRY UP ACROSS APCH AREA RWY 1L.' THERE WAS NO CONFLICT WITH ARRIVING OR DEPARTING ACFT.
