Narrative:

I thought when we pushed off the gate in sat that the cockpit door was locked and secured. However, on takeoff roll, the cockpit door came open. Because of the aircraft speed at the time the cockpit opened, the takeoff was continued. Once airborne, the door was closed by the flight attendant(south). At a safe altitude on climbout I relatched and secured the cockpit door. Note: after the door was secured for pushback. I re-opened the cockpit door to discuss bag tags and to pass the flight release to he operations agent. I then re-secured the door (I thought).

Title: THE COCKPIT DOOR ON A B737 COMES OPEN ON TKOF ROLL AFTER THE PIC HAD FAILED TO PROPERLY CLOSE IT PRIOR TO TAXI FOR TKOF AT SAT, TX.

Narrative: I THOUGHT WHEN WE PUSHED OFF THE GATE IN SAT THAT THE COCKPIT DOOR WAS LOCKED AND SECURED. HOWEVER, ON TKOF ROLL, THE COCKPIT DOOR CAME OPEN. BECAUSE OF THE ACFT SPEED AT THE TIME THE COCKPIT OPENED, THE TKOF WAS CONTINUED. ONCE AIRBORNE, THE DOOR WAS CLOSED BY THE FLT ATTENDANT(S). AT A SAFE ALT ON CLIMBOUT I RELATCHED AND SECURED THE COCKPIT DOOR. NOTE: AFTER THE DOOR WAS SECURED FOR PUSHBACK. I RE-OPENED THE COCKPIT DOOR TO DISCUSS BAG TAGS AND TO PASS THE FLT RELEASE TO HE OPS AGENT. I THEN RE-SECURED THE DOOR (I THOUGHT).
