Narrative:

On dec/thu/01, I was captain on a part 121 revenue passenger flight from dsn to cvg. Upon the flight climb out, after departing runway 31, I realized (after running the climb checklist) that the seatbelt sign was left in the off position for taxi and takeoff. At this point, immediate corrective action was taken to rectify this situation (the seatbelt sign was turned on). Upon review of the events leading to this checklist deviation, they are listed below: a quick turn in dsm to depart for cvg, and while running the 'turnaround checklist,' be disrupted by fueler, flight attendant. I realize how at the point of disturbance while running the checklist, I should have started over from the beginning of the checklist.

Title: A CARJ CREW, DEPARTING DSM, NEGLECTED TO TURN THE SEATBELT SIGN ON FOR TAXI OR TKOF.

Narrative: ON DEC/THU/01, I WAS CAPT ON A PART 121 REVENUE PAX FLT FROM DSN TO CVG. UPON THE FLT CLBOUT, AFTER DEPARTING RWY 31, I REALIZED (AFTER RUNNING THE CLB CHKLIST) THAT THE SEATBELT SIGN WAS LEFT IN THE OFF POS FOR TAXI AND TKOF. AT THIS POINT, IMMEDIATE CORRECTIVE ACTION WAS TAKEN TO RECTIFY THIS SIT (THE SEATBELT SIGN WAS TURNED ON). UPON REVIEW OF THE EVENTS LEADING TO THIS CHKLIST DEV, THEY ARE LISTED BELOW: A QUICK TURN IN DSM TO DEPART FOR CVG, AND WHILE RUNNING THE 'TURNAROUND CHKLIST,' BE DISRUPTED BY FUELER, FLT ATTENDANT. I REALIZE HOW AT THE POINT OF DISTURBANCE WHILE RUNNING THE CHKLIST, I SHOULD HAVE STARTED OVER FROM THE BEGINNING OF THE CHKLIST.
