Narrative:

Due to WX and NAVAID outages, instrument approachs were being conducted to sdl VOR-a. This procedure is internally addressed in facility orders and LOA because the sdl arrival crosses the passenger departure path. Once coordinated, phx tower is responsible to ensure vertical separation between the sdl arrival and phx departures. The tower controllers failed to ensure separation between the BE99 departure and the FA50 sdl arrival. The BE99 climbing, passed 'just behind' the FA50 descending on the approach. Just dumb luck. When the supervisor asked the tower what happened, he was told that the local controller was busy with ILS traffic and 'forgot.'

THE BE99 CLBING, PASSED 'JUST BEHIND' THE FA50 DSNDING ON THE APCH. JUST DUMB LUCK. WHEN THE SUPVR ASKED THE TWR WHAT HAPPENED, HE WAS TOLD THAT THE LCL CTLR WAS BUSY WITH ILS TFC AND 'FORGOT.'
