Narrative:

During takeoff when landing gear was retracted, a loud noise and vibration were felt. Right forward nose gear door latch did not latch securely after maintenance cleaned and lubed latch. Secured forward nose gear door. Door inspection ok. The nose gear doors on this aircraft were opened to fix a hydraulic leak. The MD88 has no door position indication system for the forward nose gear doors. The door was closed prior to pushback. There is no way for crew to know if the door opened on taxi out.

Title: AN MD88 IMMEDIATELY AFTER TKOF DIVERTED DUE TO NOISE AND VIBRATION CAUSED BY THE R NOSE GEAR DOOR UNLATCHED AND OPEN.

Narrative: DURING TKOF WHEN LNDG GEAR WAS RETRACTED, A LOUD NOISE AND VIBRATION WERE FELT. R FORWARD NOSE GEAR DOOR LATCH DID NOT LATCH SECURELY AFTER MAINT CLEANED AND LUBED LATCH. SECURED FORWARD NOSE GEAR DOOR. DOOR INSPECTION OK. THE NOSE GEAR DOORS ON THIS ACFT WERE OPENED TO FIX A HYD LEAK. THE MD88 HAS NO DOOR POS INDICATION SYS FOR THE FORWARD NOSE GEAR DOORS. THE DOOR WAS CLOSED PRIOR TO PUSHBACK. THERE IS NO WAY FOR CREW TO KNOW IF THE DOOR OPENED ON TAXI OUT.
