Narrative:

Our departure clearance from hnl to sfo was cleared to sfo as filed, on departure, fly the MKK4, cluts transition, expect FL360, without an initial departure altitude assigned. The MKK4 has a 5000 ft restriction, but this is the only airport that assigns a SID without giving an initial altitude assignment. My captain and several others have misunderstood this clearance to be climb and maintain FL360 vs the SID restr 5000 ft. I would suggest you have hnl clearance include climb and maintain 5000 ft, expect fl XXX in their clearance to avoid this confusion as well as an altitude deviation. Callback conversation with reporter revealed the following information: reporter had noted more than a few crews had confusion over the issue. He always makes it a point to stress the restriction. He thinks it could be made more evident.

Title: B767-300 FO OBSERVES SOME CAPTS DO NOT NOTE THE 5000 FT ALT RESTRICTION ON THE MOLOKAI FOUR SID AT HNL.

Narrative: OUR DEP CLRNC FROM HNL TO SFO WAS CLRED TO SFO AS FILED, ON DEP, FLY THE MKK4, CLUTS TRANSITION, EXPECT FL360, WITHOUT AN INITIAL DEP ALTITUDE ASSIGNED. THE MKK4 HAS A 5000 FT RESTRICTION, BUT THIS IS THE ONLY ARPT THAT ASSIGNS A SID WITHOUT GIVING AN INITIAL ALTITUDE ASSIGNMENT. MY CAPT AND SEVERAL OTHERS HAVE MISUNDERSTOOD THIS CLRNC TO BE CLB AND MAINTAIN FL360 VS THE SID RESTR 5000 FT. I WOULD SUGGEST YOU HAVE HNL CLRNC INCLUDE CLB AND MAINTAIN 5000 FT, EXPECT FL XXX IN THEIR CLRNC TO AVOID THIS CONFUSION AS WELL AS AN ALTITUDE DEVIATION. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR HAD NOTED MORE THAN A FEW CREWS HAD CONFUSION OVER THE ISSUE. HE ALWAYS MAKES IT A POINT TO STRESS THE RESTRICTION. HE THINKS IT COULD BE MADE MORE EVIDENT.
