Narrative:

At approximately XA28 on jun/sun/01, I received a notification from maintenance that MEL was opened on aircraft for flight to destination. I did not receive a call for authority/authorized on this placard, and on further investigation, I found that this placard was placed on this aircraft in zza on trip jun/sat/01. The next leg this aircraft operated (on jun/sat/01) it had a placard with restrs. Both of these MEL's are missing on subsequent flts. It looks like MEL was added in zzb, removed and changed in zzc, but not entered into the system until I received the above referenced notification. 3 flts were operated without the proper performance restrs applied.

Title: A MEDIUM LARGE TRANSPORT WAS OPERATED FOR 3 FLTS IN NON COMPLIANCE WITH AN AIRFOIL ANTI-ICE DEFERRAL WITHOUT THE PERFORMANCE PENALTIES APPLIED.

Narrative: AT APPROX XA28 ON JUN/SUN/01, I RECEIVED A NOTIFICATION FROM MAINT THAT MEL WAS OPENED ON ACFT FOR FLT TO DEST. THE NEXT LEG THIS ACFT OPERATED (ON JUN/SAT/01) IT HAD A PLACARD WITH RESTRS. BOTH OF THESE MEL'S ARE MISSING ON SUBSEQUENT FLTS. IT LOOKS LIKE MEL WAS ADDED IN ZZB, REMOVED AND CHANGED IN ZZC, BUT NOT ENTERED INTO THE SYS UNTIL I RECEIVED THE ABOVE REFED NOTIFICATION. 3 FLTS WERE OPERATED WITHOUT THE PROPER PERFORMANCE RESTRS APPLIED.
