Narrative:
Captain failed to comply with company policies. Deliberate failure to adhere to required checklists. After several attempts to urge the captain to comply, and his refusal, I removed myself from the trip.

Title: MD80 FO REMOVED HIMSELF FROM FURTHER SCHEDULED FLT AFTER THE CAPT REPEATEDLY WOULD NOT COMPLY WITH, OR FOLLOW, COMPANY POLICIES OR THE ACFT CHKLISTS.
Narrative: CAPT FAILED TO COMPLY WITH COMPANY POLICIES. DELIBERATE FAILURE TO ADHERE TO REQUIRED CHKLISTS. AFTER SEVERAL ATTEMPTS TO URGE THE CAPT TO COMPLY, AND HIS REFUSAL, I REMOVED MYSELF FROM THE TRIP.
