Narrative:

The first officer did not set takeoff flaps on after start flow. While taxiing, we were interrupted 2 times while reading the before takeoff checklist. The checklist was not finished. While advancing the thrust levers (first officer takeoff) the takeoff warning horn sounded. The first officer stopped the aircraft and selected flaps 5 degrees. The captain read the before takeoff checklist and we continued the takeoff with 9500 ft of runway remaining. I believe that the 2 MEL's, maintenance pulling the ground power without the APU running were factors in my being distraction. I also had 2 poor nights of sleep in a row. My wife was upset that my son fell and needed stitches and I was not home. Also I was going to pit to see my cousin in intensive care. I believe this is a good wake-up call for me and it will not happen again.

Title: B737-300 CREW DID NOT SET TKOF FLAPS AT GSO.

Narrative: THE FO DID NOT SET TKOF FLAPS ON AFTER START FLOW. WHILE TAXIING, WE WERE INTERRUPTED 2 TIMES WHILE READING THE BEFORE TKOF CHKLIST. THE CHKLIST WAS NOT FINISHED. I BELIEVE THAT THE 2 MEL'S, MAINT PULLING THE GND PWR WITHOUT THE APU RUNNING WERE FACTORS IN MY BEING DISTR. I ALSO HAD 2 POOR NIGHTS OF SLEEP IN A ROW. MY WIFE WAS UPSET THAT MY SON FELL AND NEEDED STITCHES AND I WAS NOT HOME. ALSO I WAS GOING TO PIT TO SEE MY COUSIN IN INTENSIVE CARE. I BELIEVE THIS IS A GOOD WAKE-UP CALL FOR ME AND IT WILL NOT HAPPEN AGAIN.
