Narrative:

At rotation, a slight vibration was felt with loss of right side hydraulic system pressure and quantity. A turn to downwind was initiated while complying with hydraulic system failure checklist. Subsequently, right side thrust reverse light 'unlocked' illuminated followed by right reverse light on steady. Right engine brought to idle power, emergency declared, aircraft stopped on runway and right engine secured. Aircraft towed to gate. Callback conversation with reporter revealed the following information: the reporter stated the engine was pulled back to idle power with the reverser light on continuously. The reporter said the engine was never in reverse and was only a false warning caused by the engine vibrations shaking the reverser position indication sensors and wiring. The reporter stated the cause of the right engine loss of power is unknown as the engine shop has not started the tear-down.

Title: AN MD88 SHORTLY AFTER ROTATION DECLARED AN EMER AND DIVERTED DUE TO R ENG VIBRATION, LOSS OF PWR, AND HYD SYS FAILURE.

ACFT TOWED TO GATE. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE ENG WAS PULLED BACK TO IDLE PWR WITH THE REVERSER LIGHT ON CONTINUOUSLY. THE RPTR SAID THE ENG WAS NEVER IN REVERSE AND WAS ONLY A FALSE WARNING CAUSED BY THE ENG VIBRATIONS SHAKING THE REVERSER POS INDICATION SENSORS AND WIRING. THE RPTR STATED THE CAUSE OF THE R ENG LOSS OF PWR IS UNKNOWN AS THE ENG SHOP HAS NOT STARTED THE TEAR-DOWN.
