Narrative:
Upon arrival at bna, I realized that the following PIREP was not signed off at lga. Seatbelt sign at seats xa and xb are inoperative. I was aware that this item was in the book as I reviewed the log upon reaching the aircraft. Factors involved in my lapse were: a long day, a very new first officer. Maintenance was advised of the open item upon arrival at bna.

Narrative: UPON ARR AT BNA, I REALIZED THAT THE FOLLOWING PIREP WAS NOT SIGNED OFF AT LGA. SEATBELT SIGN AT SEATS XA AND XB ARE INOP. I WAS AWARE THAT THIS ITEM WAS IN THE BOOK AS I REVIEWED THE LOG UPON REACHING THE ACFT. FACTORS INVOLVED IN MY LAPSE WERE: A LONG DAY, A VERY NEW FO. MAINT WAS ADVISED OF THE OPEN ITEM UPON ARR AT BNA.
