Narrative:

Runway braking action was reported fair to poor. The first officer made the takeoff. At approximately 100 KTS, a fire warning bell chirped then ceased. I noted the cargo fire light flicker then go out. The takeoff was continued due to the high speed and slippery conditions. After takeoff, the cabin would not pressurize. Shortly thereafter, a flight attendant called to report the aft cargo door was open. We leveled off at 4000 ft and notified approach of our intention to return to syr. We completed all normal checklists and the door open checklist. ATC asked if we wanted the emergency equipment and we responded yes. We flew an ILS approach to landing and taxied clear. The emergency vehicles reported no damage so we returned to the gate. 3 ramp personnel reported the door was closed prior to taxi and no one on/in the deice truck reported anything unusual. On return to the gate, the door lever was down and locked, which according to contract maintenance was not possible. No cargo was lost and no one was injured.
