Narrative:

Taxi instructions received were taxiway B, taxiway G, taxiway east, runway 19R. Joined taxiway east, waited behind 2 aircraft at taxiway V. One aircraft departed from taxiway V on runway 19R. Next departure was from full length runway 19R. Third departure was aircraft from taxiway V on runway 19R. We proceeded down taxiway east to full length runway 19R. Crossing runway 25R, tower asked 'did you want full length?' we (first officer) responded 'affirmative.' tower queried 'did ground give you full length?' first officer responded 'we got taxiway B, taxiway G, taxiway east, runway 19R.' no further instructions until cleared into position full length runway 19R. After departure, I listened to ATIS which had changed with departures off runway 19R at taxiway V, and runways 25L&right were reported in use. We were not given a hold short clearance with ground, nor expected departure at taxiway V. Ground advised us to switch to tower when we were joining taxiway east, and no restrs were issued. There were no aircraft visible utilizing runway 25R when we crossed, but we felt that the tower controller was surprised when we proceeded to full length. Possible 'dropped ball.' 1) flight crew not obtaining/receiving new ATIS or advised of change. 2) ground control not informing flight crew of departures from taxiway V. 3) tower controller assurance all aircraft received taxiway V from ground control.

Title: MD80 CREW HAD ATCT GND AND ATCT LCL CTLR MISHANDLING AT MKE.

Narrative: TAXI INSTRUCTIONS RECEIVED WERE TXWY B, TXWY G, TXWY E, RWY 19R. JOINED TXWY E, WAITED BEHIND 2 ACFT AT TXWY V. ONE ACFT DEPARTED FROM TXWY V ON RWY 19R. NEXT DEP WAS FROM FULL LENGTH RWY 19R. THIRD DEP WAS ACFT FROM TXWY V ON RWY 19R. WE PROCEEDED DOWN TXWY E TO FULL LENGTH RWY 19R. XING RWY 25R, TWR ASKED 'DID YOU WANT FULL LENGTH?' WE (FO) RESPONDED 'AFFIRMATIVE.' TWR QUERIED 'DID GND GIVE YOU FULL LENGTH?' FO RESPONDED 'WE GOT TXWY B, TXWY G, TXWY E, RWY 19R.' NO FURTHER INSTRUCTIONS UNTIL CLRED INTO POS FULL LENGTH RWY 19R. AFTER DEP, I LISTENED TO ATIS WHICH HAD CHANGED WITH DEPS OFF RWY 19R AT TXWY V, AND RWYS 25L&R WERE RPTED IN USE. WE WERE NOT GIVEN A HOLD SHORT CLRNC WITH GND, NOR EXPECTED DEP AT TXWY V. GND ADVISED US TO SWITCH TO TWR WHEN WE WERE JOINING TXWY E, AND NO RESTRS WERE ISSUED. THERE WERE NO ACFT VISIBLE UTILIZING RWY 25R WHEN WE CROSSED, BUT WE FELT THAT THE TWR CTLR WAS SURPRISED WHEN WE PROCEEDED TO FULL LENGTH. POSSIBLE 'DROPPED BALL.' 1) FLC NOT OBTAINING/RECEIVING NEW ATIS OR ADVISED OF CHANGE. 2) GND CTL NOT INFORMING FLC OF DEPS FROM TXWY V. 3) TWR CTLR ASSURANCE ALL ACFT RECEIVED TXWY V FROM GND CTL.
