Narrative:

On oct/wed/00, I received a letter of investigation regarding aircraft xyz and damage to its l-hand wing to body fillet panels. This letter contained information that I was previously unaware of. This response letter contains an explanation of events I was aware of at the time of my release for maintenance ferry to XXX for repairs. I have also attached the letter of investigation, log reports, spotting message, and logbook release. Callback conversation with reporter revealed the following information: the reporter stated the aircraft arrived in ZZZ with a damaged left wing fuselage fillet hole and crushed composite and a cut in the left main gear door. The reporter said he was working as a shift supervisor and was given a turnover that the aircraft was ready for maintenance ferry to XXX. The reporter stated he signed the logbook maintenance release and maintenance ferry inspection release and the aircraft was dispatched. The reporter said the FAA safety inspector was looking into the paperwork and found several undocumented repairs that were unknown and not turned over from the previous shift. The reporter said the previous shift had removed the fillet and had a composite repair working when a maintenance manager ordered the repair stopped and the fillet put on the airplane uncompleted with no write-ups. The reporter said the taping of the fillet an interim repair ordered by the maintenance controller was not documented and the FAA questioned the lack of a write-up. The reporter stated none of the attempted fillet repair or fillet taping interim repair was documented or included in the shift turnover. The reporter stated the airplane was dispatched without the maintenance controller entering a deferred item requiring repair of the fillet and door at termination of the maintenance ferry.

Title: AN MD80 WAS DISPATCHED ON A MAINT FERRY IN NON-COMPLIANCE WITH UNCORRECTED DOCUMENTATION DISCREPANCIES.

Narrative: ON OCT/WED/00, I RECEIVED A LETTER OF INVESTIGATION REGARDING ACFT XYZ AND DAMAGE TO ITS L-HAND WING TO BODY FILLET PANELS. THIS LETTER CONTAINED INFO THAT I WAS PREVIOUSLY UNAWARE OF. THIS RESPONSE LETTER CONTAINS AN EXPLANATION OF EVENTS I WAS AWARE OF AT THE TIME OF MY RELEASE FOR MAINT FERRY TO XXX FOR REPAIRS. I HAVE ALSO ATTACHED THE LETTER OF INVESTIGATION, LOG RPTS, SPOTTING MESSAGE, AND LOGBOOK RELEASE. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE ACFT ARRIVED IN ZZZ WITH A DAMAGED L WING FUSELAGE FILLET HOLE AND CRUSHED COMPOSITE AND A CUT IN THE L MAIN GEAR DOOR. THE RPTR SAID HE WAS WORKING AS A SHIFT SUPVR AND WAS GIVEN A TURNOVER THAT THE ACFT WAS READY FOR MAINT FERRY TO XXX. THE RPTR STATED HE SIGNED THE LOGBOOK MAINT RELEASE AND MAINT FERRY INSPECTION RELEASE AND THE ACFT WAS DISPATCHED. THE RPTR SAID THE FAA SAFETY INSPECTOR WAS LOOKING INTO THE PAPERWORK AND FOUND SEVERAL UNDOCUMENTED REPAIRS THAT WERE UNKNOWN AND NOT TURNED OVER FROM THE PREVIOUS SHIFT. THE RPTR SAID THE PREVIOUS SHIFT HAD REMOVED THE FILLET AND HAD A COMPOSITE REPAIR WORKING WHEN A MAINT MGR ORDERED THE REPAIR STOPPED AND THE FILLET PUT ON THE AIRPLANE UNCOMPLETED WITH NO WRITE-UPS. THE RPTR SAID THE TAPING OF THE FILLET AN INTERIM REPAIR ORDERED BY THE MAINT CTLR WAS NOT DOCUMENTED AND THE FAA QUESTIONED THE LACK OF A WRITE-UP. THE RPTR STATED NONE OF THE ATTEMPTED FILLET REPAIR OR FILLET TAPING INTERIM REPAIR WAS DOCUMENTED OR INCLUDED IN THE SHIFT TURNOVER. THE RPTR STATED THE AIRPLANE WAS DISPATCHED WITHOUT THE MAINT CTLR ENTERING A DEFERRED ITEM REQUIRING REPAIR OF THE FILLET AND DOOR AT TERMINATION OF THE MAINT FERRY.
