Narrative:

I was notified by the fueling supervisor that the above aircraft had a fuel imbal condition, too much fuel in right main tank. I sent technician wxxz out to investigate. The right main tank had 10200 pounds, left tank had about 4300 pounds and center tank had about 6400 pounds. I went out and verified fuel onboard and watched wxxz xfer fuel from right tank to center and left tank until we were left with 9200 pounds in right tank. Fuel xfer was normal, right tank decreased in 50 pound increments, and center and left tank increased the same. Total fuel quantity remained the same. The quantity indicator system was tested by flight crew before fuel xfer and found to be normal. The aircraft was fueled to its fuel load, left main 9200 pounds, center 6800 pounds, right 9200 pounds. Flight crew again tested fuel quantity with no discrepancies. The aircraft departed and air interrupted with fuel imbal. Right main dropped to 2600 pounds on climb. When aircraft returned right tank was placarded according to MEL.

Title: AN MD80 WAS DISPATCHED IN NON COMPLIANCE WITH A 6600 LB FUEL IMBAL CAUSED BY IMPROPER FUELING PROCS.

Narrative: I WAS NOTIFIED BY THE FUELING SUPVR THAT THE ABOVE ACFT HAD A FUEL IMBAL CONDITION, TOO MUCH FUEL IN R MAIN TANK. I SENT TECHNICIAN WXXZ OUT TO INVESTIGATE. THE R MAIN TANK HAD 10200 LBS, L TANK HAD ABOUT 4300 LBS AND CTR TANK HAD ABOUT 6400 LBS. I WENT OUT AND VERIFIED FUEL ONBOARD AND WATCHED WXXZ XFER FUEL FROM R TANK TO CTR AND L TANK UNTIL WE WERE LEFT WITH 9200 LBS IN R TANK. FUEL XFER WAS NORMAL, R TANK DECREASED IN 50 LB INCREMENTS, AND CTR AND L TANK INCREASED THE SAME. TOTAL FUEL QUANTITY REMAINED THE SAME. THE QUANTITY INDICATOR SYS WAS TESTED BY FLC BEFORE FUEL XFER AND FOUND TO BE NORMAL. THE ACFT WAS FUELED TO ITS FUEL LOAD, L MAIN 9200 LBS, CTR 6800 LBS, R 9200 LBS. FLC AGAIN TESTED FUEL QUANTITY WITH NO DISCREPANCIES. THE ACFT DEPARTED AND AIR INTERRUPTED WITH FUEL IMBAL. R MAIN DROPPED TO 2600 LBS ON CLB. WHEN ACFT RETURNED R TANK WAS PLACARDED ACCORDING TO MEL.
