Narrative:

Far ATIS received at the gate prior to taxi was: wind 280 degrees/3 KTS, 5 SM visibility, sky clear, temperature 18 degrees/dewpoint 17 degrees, altimeter 30.00. On taxi to runway ground control reported the RVR at 5000 ft. Tower control cleared us into position and hold. As we taxied into position tower control then reported RVR at 1600 ft. As we stopped in position on the runway tower control reported RVR at 1200 ft and asked if we could go. We responded that we would have to look it up. Having the 'go' mindset once cleared into position and hold on the runway and seeing in the fom (flight operations manual) 'takeoff minimums lower than RVR 1200 ft...are only authority/authorized...under the following conditions: a) the runway is equipped with operative centerline lights...' this led us to believe that we met the RVR requirements for takeoff on a runway not equipped with centerline lights. After further review of our operations specifications revealed that 1600 ft RVR was required, and that the fom section on 'takeoff minimums-RVR' did not reflect the requirements for centerline lights. A 'manual change request form' has been submitted to our flight department to have the fom agree with the operations specifications. Although briefly considered, the requirement for a takeoff alternate was completely forgotten in our discussion of takeoff minimums.

AS WE STOPPED IN POS ON THE RWY TWR CTL RPTED RVR AT 1200 FT AND ASKED IF WE COULD GO. WE RESPONDED THAT WE WOULD HAVE TO LOOK IT UP. HAVING THE 'GO' MINDSET ONCE CLRED INTO POS AND HOLD ON THE RWY AND SEEING IN THE FOM (FLT OPS MANUAL) 'TKOF MINIMUMS LOWER THAN RVR 1200 FT...ARE ONLY AUTH...UNDER THE FOLLOWING CONDITIONS: A) THE RWY IS EQUIPPED WITH OPERATIVE CTRLINE LIGHTS...' THIS LED US TO BELIEVE THAT WE MET THE RVR REQUIREMENTS FOR TKOF ON A RWY NOT EQUIPPED WITH CTRLINE LIGHTS. AFTER FURTHER REVIEW OF OUR OPS SPECS REVEALED THAT 1600 FT RVR WAS REQUIRED, AND THAT THE FOM SECTION ON 'TKOF MINIMUMS-RVR' DID NOT REFLECT THE REQUIREMENTS FOR CTRLINE LIGHTS.
