Narrative:
Distracted by malfunctioning DME display. Forgot to reset transponder to 1200 ft after arrival at ues. Flew from ues to ugn with code I got inbound to ues from ugn that I got from mke approach for advisories. I did not contact approach for the trip from ues to ugn.

Narrative: DISTRACTED BY MALFUNCTIONING DME DISPLAY. FORGOT TO RESET XPONDER TO 1200 FT AFTER ARR AT UES. FLEW FROM UES TO UGN WITH CODE I GOT INBOUND TO UES FROM UGN THAT I GOT FROM MKE APCH FOR ADVISORIES. I DID NOT CONTACT APCH FOR THE TRIP FROM UES TO UGN.
