Narrative:

I was the so aboard air carrier flight (B747-200FA) from jfk to ord and terminating in anc. Dangerous goods in the form of radioactive material was loaded in jfk to transport to anc. The required notice to PIC forms were properly filled out and provided to the flight crew. Upon arrival at ord, substantially more dangerous goods were loaded for transport to anc and additional notice to PIC forms provided. The freight agent at ord personally notified me of the fact that the radioactive material brought from jfk had been moved to a new location on the cargo deck and he escorted me throughout my cargo deck inspection to confirm the location of all dangerous goods. All necessary forms were aboard flight upon departure from ord. Upon arrival at anc, the freight agent came aboard and took possession of all dangerous goods paperwork. I was later notified by air carrier safety that the form for the radioactive material loaded in jfk could not be found at anc. I am confident that all proper procedures were followed by the flight and that each freight agent acted in accordance with company dangerous goods policy. I have no explanation for the allegedly missing radioactive material form.

Title: REQUIRED HAZARDOUS MATERIALS PAPERWORK LOST.

Narrative: I WAS THE SO ABOARD ACR FLT (B747-200FA) FROM JFK TO ORD AND TERMINATING IN ANC. DANGEROUS GOODS IN THE FORM OF RADIOACTIVE MATERIAL WAS LOADED IN JFK TO TRANSPORT TO ANC. THE REQUIRED NOTICE TO PIC FORMS WERE PROPERLY FILLED OUT AND PROVIDED TO THE FLC. UPON ARR AT ORD, SUBSTANTIALLY MORE DANGEROUS GOODS WERE LOADED FOR TRANSPORT TO ANC AND ADDITIONAL NOTICE TO PIC FORMS PROVIDED. THE FREIGHT AGENT AT ORD PERSONALLY NOTIFIED ME OF THE FACT THAT THE RADIOACTIVE MATERIAL BROUGHT FROM JFK HAD BEEN MOVED TO A NEW LOCATION ON THE CARGO DECK AND HE ESCORTED ME THROUGHOUT MY CARGO DECK INSPECTION TO CONFIRM THE LOCATION OF ALL DANGEROUS GOODS. ALL NECESSARY FORMS WERE ABOARD FLT UPON DEP FROM ORD. UPON ARR AT ANC, THE FREIGHT AGENT CAME ABOARD AND TOOK POSSESSION OF ALL DANGEROUS GOODS PAPERWORK. I WAS LATER NOTIFIED BY ACR SAFETY THAT THE FORM FOR THE RADIOACTIVE MATERIAL LOADED IN JFK COULD NOT BE FOUND AT ANC. I AM CONFIDENT THAT ALL PROPER PROCS WERE FOLLOWED BY THE FLT AND THAT EACH FREIGHT AGENT ACTED IN ACCORDANCE WITH COMPANY DANGEROUS GOODS POLICY. I HAVE NO EXPLANATION FOR THE ALLEGEDLY MISSING RADIOACTIVE MATERIAL FORM.
