Narrative:

Stabilizer trim failure. Flight abc departed phx on time. Passing 10000 ft, I noticed a problem with the stabilizer trim. It would not work. The captain confirmed his trim would not work either. Leveled at 13000 ft. The trim began to work again. Initiated climb again and the stabilizer trim failed again. We discontinued the flight, called ATC, declared an emergency, ran the appropriate checklists and landed back at phx. Wrote the incident up in the logbook.

Title: MD80 CREW HAD THE STABILIZER TRIM SYS FAIL ON DEP AT PHX.

Narrative: STABILIZER TRIM FAILURE. FLT ABC DEPARTED PHX ON TIME. PASSING 10000 FT, I NOTICED A PROB WITH THE STABILIZER TRIM. IT WOULD NOT WORK. THE CAPT CONFIRMED HIS TRIM WOULD NOT WORK EITHER. LEVELED AT 13000 FT. THE TRIM BEGAN TO WORK AGAIN. INITIATED CLB AGAIN AND THE STABILIZER TRIM FAILED AGAIN. WE DISCONTINUED THE FLT, CALLED ATC, DECLARED AN EMER, RAN THE APPROPRIATE CHECKLISTS AND LANDED BACK AT PHX. WROTE THE INCIDENT UP IN THE LOGBOOK.
