Narrative:

Crew accepted aircraft with an open write-up (maintenance discrepancy) 'yaw damper inoperative.' crew advised company maintenance of problem and it was properly reviewed and placed on the MEL list. Crew followed proper MEL procedure but failed to ensure that the MEL was placed on the dispatch release prior to departure. Approximately 20 mins into flight, crew realized mistake and promptly informed dispatch. Dispatch acknowledged through ACARS.

Title: A DC9-30 WAS DISPATCHED IN NON COMPLIANCE WITH THE YAW DAMPER DEFERRED AS INOP PER THE MEL BUT DISPATCH WAS NOT ADVISED.

Narrative: CREW ACCEPTED ACFT WITH AN OPEN WRITE-UP (MAINT DISCREPANCY) 'YAW DAMPER INOP.' CREW ADVISED COMPANY MAINT OF PROB AND IT WAS PROPERLY REVIEWED AND PLACED ON THE MEL LIST. CREW FOLLOWED PROPER MEL PROC BUT FAILED TO ENSURE THAT THE MEL WAS PLACED ON THE DISPATCH RELEASE PRIOR TO DEP. APPROX 20 MINS INTO FLT, CREW REALIZED MISTAKE AND PROMPTLY INFORMED DISPATCH. DISPATCH ACKNOWLEDGED THROUGH ACARS.
