Narrative:

I gave clearance to close #1 and #2 engine fan cowls. Deactivation pins for reversers were still installed. Nobody documented that pins were installed per our maintenance requirements. Reversers did not deploy for test pilots. The card for removing these pins was accomplished prior to the first test flight the day before. The pins were reinstalled without documentation. Our policy for clearance does not look for deactivated engine components.

Title: AN AIRBUS 320 ON COMPLETING A TEST FLT AND ON LNDG ROLLOUT BOTH ENG REVERSERS FAILED TO DEPLOY CAUSED BY FAILURE TO REMOVE REVERSER LOCK PINS AFTER MAINT.

Narrative: I GAVE CLRNC TO CLOSE #1 AND #2 ENG FAN COWLS. DEACTIVATION PINS FOR REVERSERS WERE STILL INSTALLED. NOBODY DOCUMENTED THAT PINS WERE INSTALLED PER OUR MAINT REQUIREMENTS.
